Screening Tool for Disability Discrimination

Description

Two brand new screening tools were created as part of the Tips to Help Health Centers Address Disability & Chronic Disease Discrimination resource. The tools help healthcare providers determine if a patient is experiencing discrimination based on a disability or chronic disease. There is one tool for adults and another for children and young adults (including those attending college).
